Very rare four-sided Jewish document (questionnaire) given by the Germans in 1942 to residents of formerly Soviet-occupied Poland intended to identify Jewish professionals apparently with a view to their impending deportation to camps. The document to be completed for the German authorities by health professionals reporting for the first time (Fragebogen zur erstmaligen Meldung der Heilberufe) was filled out by Saul Arno Gruenberg, a dental technician from the town of **Kolomea (Kolomyia Polish) Eastern Galicia. (This document has been fully filled out).
His photograph has been affixed to the document with a Red Star of David stamped on the top center of the front page. On the inside of the first page, it mentions his parents and grandparents by name with the addition 'Jude' and 'Judin'.
This rare historical four-sided document measures 8.5 x 12.25ins (22 x 31cm). It is in excellent condition and has been neatly folded in half at some time in the past.
