מסמך נאצי רשמי, כרטיס מזון אישי. עם סמל עיט הרייך ומתחתיו צלב הקרס. אפריל, 1940.
בתעודה פירוט של סוגי המזון המתקבלים באמצעותה ממשרד התזונה. פרטי בעלת התעודה מולאו בכתב יד (שם וכתובת), כשמתחתיהם מופיעים הגבלות והתניות כגון איסור העברת התעודה לאדם אחר. התעודה הייתה תקפה למשך כחודש ימים החל מ-8 באפריל ועד 5 במאי 1940.
